It's basically sharing the bill with your insurance. They cover X, you have Y, which can be percentages (like 80/20 with Medicare) or Flat rate (300 for a visit) etc etc.
American health care is convoluted exactly for corporate benefits
Technically, you're conflating copay and coinsurance.
Copay is a flat rate you pay per visit, based on type of facility.
Coinsurance is a % you pay after your deductible is finished, usually also based on the type of facility.
Some policies have an out of pocket maximum that puts a cap on your yearly out of pocket payments, but there's a lot that can go wrong with those (like facilities failing to refund you when they're supposed to)
Also right. But a lot of people don't understand coinsurance (80/20 split example above) and copay (flat rate) and how it all comes together for the valuation of estimates and billing.
For example: Policies also negotiate a cap on prices for services rendered which is where our coinsurance %s are based off of but onky after any deductables (100% Patient Owned Liability that must be billed to or paid for by the patient before insurance even begins to cover any costs while under this plan) and co pay flat values are considered first.
So if a plan has a 2K deductable available, the maximum charge allowed for the visit or services performed is 3K with the plans negotiated or standard rate, and the insurance plan is a 20% coinsurance and a $500 copay, the expected bill would break down to:
Maximum cost: 3K Dedutible: 2K Copay: 500 Coinsurance: 20%
And for ahits and grins lets say theres a max Out of Pocket (OOP) of 5K.
3000-2000-500= 500 remaining balance that can be split with the Coinsurance, adding another 100 meaning the expected bill would be Deductable+Copay+Coinsurance and valued against Maximum Allowed Amount and OOP values to make sure that the value received, in this case the total would be $2600 of a $3000 procedure, and leave an Out of Pocket Balance amount of $2400 for any future visits.
The other rub most people are unaware of is that these pools reset every year on the policy's anniversary, meaning that those values are what you have to consider every year as potential medical expenses. Once you hit the maximum OOP value, insurance will be 100% responsible for any future medical bills until the next yearly refresh of your policy values.
And I have to explain that specific breakdown daily for people so they understand how, despite them having insurance, a visit to the ED where your negotaiated policy rates are not as low as the other hospital you may go to, and end up with a bill that looks like you have no coverage at all (despite the fact that originally your ED bill would have been 2x that, but they don't see the Original Cost, just the Max allowed value).
Also, if you have to have several visits or procedures that are billed under seperate visits and not able to be bunsled (such as being seen in two different EDs even if it is the same problem but not for transfer purposes. I.e. it is definitely two aeperate visits), Deductables and co-pays may not be accurately reflected as the bill for the first or prior visit in the chain may not have actually been fully aubmitted as a claim which when another pwrson runs any claim against it will not realize that they may be violating those caps, leading to headaches and concerns regarding bills or estimates rendered that the patient is unable or unwilling to discuss or pay for.
And that's just from my experience in my hospital, a relatively smaller operation in a fairly populated area here and the fact that this example was just one fictious example with simple, easy rounded numbers and vaguery on the medical side of it.
Anyways, to loop all the way back around to the beginning, this is why I sloppily mixed Copay and CoInsurance in kind of the same boat, because at the end of all of it, most people only understand it (as far as I've observed) as just 'Co-pays' or 'The Bill'.

In all instances except either a high subscription I mean monthly premium plan or actual real emergencies...mostly.
