The Totenkopf is pretty much hard-wired into western symbolism as being connected to the SS.
The SS-Totenkopfverbände ("Death's Head Units") were the branch of the SS responsible for the running of the Nazi death camps.
I think the symbol represents something that is about as far from Christ as it's possible to be, and it inappropriate to be seen anywhere other than historical contexts.
I would heavily advise against wearing this symbol.

That specific stylization of the skull looks nearly identical to the "Totenkopf" used by the Schutzstaffel in Nazi Germany. Do not wear this symbol, and advise your friend to remove iconography of an evil regime that actively opposed and persecuted Christ‘s church from their website.

I have a crucifix with a skull under the cross to symbolize Golgotha but this isn’t that.
I wouldn’t wear that anywhere because it is way too similar to the SS Totenkopf of Nazi Germany. I think it may actually be a copy and paste of the literal Nazi symbol. That’s a hard no for me even if it isn’t a sin.

While skulls and memento mori are a thing, that is an actual Nazi symbol. You might cause a scandal which, if I remember correctly, is a sin. It might lead others to question our Church. This might also put you in danger, physically

Normally skulls in Christianity is a symbol and reminder of what sin leads to, usually very cool and not at all sinful. I think one of the saints used to preach while holding one.

The Skull (SS-Totenkopf) is a symbol of one of the most gruesome and merciless divisions of the Nazis. They oversaw the concentration camps and were responsible for countless deaths, rapes, torture, and other horrors. You asked whether this would be sacrilegious... I can, in fact, imagine nothing which is further from God. Your friend is an ahistorical idiot and probably a misanthropic ideologue. It has nothing to do with Catholicism. Maybe educate yourself on the role of the Catholic Church during the NS times. would you wear a Swastika with "Christ is Kind"... just as absurd and perverse.

Jesus is life and all things good. This hat looks like blasphemy to me because it seems to imply that Jesus is death and evil doing (given the use of the skull and bones in the past). This makes me feel very uneasy for your friend.

I would advise even atheists to not wear anything to do with death, witchcraft, alcohol, drugs, lust of any kind, idolatry, buddha etc... God is in control, we live for him in this life so that we can continue living on afterwards with him. Staying away from those things, helps us grow closer to God. Jesus is the only way.

I don’t think it’s a sin in and of itself.
As others have pointed out, that skull is the Totenkopf which was used by the SS.
Detachments of the SS committed some pretty awful war crimes and the Totenkopfverbande (that symbol’s namesake) administered concentration camps. People who recognize the symbol may give you shit, and the SS had unchristian beliefs and practices pretty much any way you skew it.
All that said, I think the hat is dope and reclaiming symbols/using them in different contexts (with exceptions) is cool. If you’re not wearing it as an endorsement of the SS’ less than moral activities, and the hat honors Christ’s name, it’s not a sin. May not be wise depending on what people in your neighborhood are like tho.

Considering the historical usage of the skull, I think at the very least the majority of people would interpret it very negatively.
