There is no easy way. It needs to be removed and replaced.

How big are those tiles? Large format tiles are prone to stress cracks on cuts that have 90 degree corners on the cut. The should have a small rounded but (edit-bit) in the corners of the cut.

Tile setter here. Yes. You need to remove the hardwood and ideally the old subfloor( if it’s the old 6” boards run diagonally across the joists it has to go. It does not meet modern standards anymore) replace it with new 3/4” subfloor then a second layer of subfloor(plywood-not great but acceptable, cement board- better but watch some videos on how to do it properly or uncoupling membrane,also watch videos on how to do that properly
