If David Garrison left the show because he missed live theatre, why did he keep coming back? And why did he come back for an episode that was a backdoor pilot for a spinoff?

He came back for his first three guest episodes because he could fit them into his schedule. He had time enough to do theatre and an episode of TV every once in a while. He didn't have enough time to do both theatre and appear in over twenty episodes of a show each year.
The thing about pilots that don't get picked up is we don't know how much would have changed if it had. Maybe Steve would have been an antagonist who popped up in only a few episodes a season, so they could film the scenes he appeared in when it worked for him schedule wise. Maybe his character would have been recast or just written out after a few episodes. Maybe he agreed to do it rightly figuring it wouldn't get picked up as a series.

Idk, I think had he really regret leaving, they could’ve wrote him back into the show, somehow.
I could see him and Jefferson having a rivalry at first, but then they become friends because they both have things they hate about Marcy.
Not unlike Alan and Herb in Two and a half men.
Single Steve would’ve been great with No Ma’am.
He wouldn’t have to have been a regular, but as a once in a while, every 2-3 times a season would’ve been great.
But either way, the few times he returned were truly memorable. Even in that back door pilot nonsense.

I think he was just bummed his career was getting derailed. Probably felt like a third wheel on Married with children.
He had regerts.
David Garrison was hired as a guest host and probably needed the money and exposure of a popular sitcom.
Like any actor who works, they want the payday.

Why wouldn't you come back? He was loved by the audience. On his return, he got the longest standing ovation that anyone ever got on the show.
It would be sweet if he popped up on the cartoon.
