If she's in medical rehab (I assume that's what the skilled nursing is?), she'll have a social worker assigned to her. Talk to them about what you're going to have to do to get her into a long term nursing home or in-home hospice care. Taking care of someone who is bedridden is basically a full time job, it's a huge ask of a family member, it's not something you want to force on anyone. If your sister decides to take both parents, make sure she's got home healthcare workers coming in to help her daily.
