Background: originally had a few large dead patches. Stuck a screwdriver in the ground and hit rocks and debris almost immediately. Dug it up, cleared as much stones as possible, new top soil and sod. Watered the heck out of it, lots of rain, that was about 2 months ago. Over the last few weeks just saw it slowly deteriorate and now looks dead. We’re planning on aerating and reseeding soon. Anything else I should do?

Are you sure it's dead? This summer has been brutal on my lawn. I have large spots that looked entirely brown and crispy after a long drought. I watered periodically to keep the grass from outright dying, but not enough to break dormancy. The areas recovered pretty well but now look like crap again. I have a lot of rain in the forecast and expect them to recover again as temps cool.
I don't think you're supposed to aerate sod until after the first season. You might be better off just irrigating and waiting it out.
Yeah don't aerate sod for a full calendar year - it's too stressful for it and the dirt soil beneath shouldn't be compacted anyway

OP go outside and grab the grass and tug on it. If it resists then it's dormant, water the shit out of it. If it tears out as you pull on the grass it's dead jim and you need to reseed

Update: grass is all back! Not dead. Some heavy rains did the trick.
You parking an RV next to it or something? That's either a reflection or there's more under the soil.

Not sure if it’s dead or if it’s gone dormant from lack of water by looking at it. Might have had trouble adhering to your soil. Fertilizer might help, if that soil wasn’t aerated properly it might be too compacted for the roots to grab. I’m no expert though and it might very well be dead
