Except it’s not trans phobic nor is it fatphobic to deny surgery to super morbidly obese people, it is much much much much harder to keep you alive on the table but they are right that it’s only really trans top surgeries that they’re denied which is definitely strange.
Seriously. This one bothers me so much. Not being able to get surgery isn't about the doctors being judgey about their weight. It's about not wanting your patient to die.

A trans man I knew had that happen to him because he couldn't be arsed to make better eating choices. I wish I could have sympathy for him but he's an asshole.
And fat is oestrogenic, so excess fat can alter your hormone levels.
Yup they’re hormonal active but part of the reason top surgery is difficult is because you’re removing the mammary glands

Surgery is quite literally gonna make your already stretched insides even smaller due to stitching.
True I did look up the complication rate and with a good surgeon it is still reasonably low even in super morbid obesity. However, and it’s a big however, trans surgery is still pretty rare and the reluctance of surgeons to perform it is understandable. Particularly if they’re on hormone replacement therapy, what I would do as their PCP is sit them down and have an honest conversation about exploring non surgical interventions for gender affirming care first and put them in touch with a dietician so they can start losing weight and down the track then they can look at top surgery. But that’s fatphobic according to them.
Even if you don't die i'd take a body that doesn't fit right over mangled meatflesh because the surgeon couldn't do their best work.
I would assume botched surgery dysphoria would be worse than gender dysphoria
Gender dysphoria is a little more complicated than that but I take your point it’s exactly right that a surgeon cannot do their best work, top surgery is more complicated than just lopping bewbs off

That is also true haha. I think we are working towards a more equitable society but I think the fat activism community provide more headwinds rather than assistance for the issue
From what i see there are 3 approaches to the "accommodation" issue.
The idealistic "magic cure" (impossible)
The "wheelchair ramp" strategy (flawed but possible)
And owning servants to carry you in a litter strategy. (Cruel)
I believe FAs would support the third because in a way they already demand it.
