Is it possible to have a device/code that can react to live sports?

You could go about this in several ways. One way would be if you knew a sport news site that has an api. Then you could poll the api for scores and when 1 increases, kick off light etc.
And most likely I could do an “if: Homerun then:activate” Command? I know that’s not directly the right code but it should be possible if the API includes homeruns/touchdowns?
Yup pretty much, and sure that's possible but a touchdown can be inferred, so on your polling, if the score went up by 6 from your last check in, boom you know a touchdown was scored. As for lights, there are bulbs with api. I once wrote an ambiance thing for my wife that linked our lights with the sunset color in the gameworld and it's music.
Your program needs some “source” of data. For example, some api that publishes sporting activity. You can consume the data from that API and do what you please with it.

Look into sports APIs, this is a simple program. I'm sure google has a sports API you can use.
