Tbf, you should only sort of need someone to hold the ladder while you’re ascending/ descending. If it’s slipping out while you’re stationary, then the ladder wasn’t set right.
Irrelevant to the situation described. If you’re asked to hold the ladder, you hold the ladder. Who cares if the other person would be fine for some of the time, you have one job that takes minimal focus and effort. Just hold the ladder.
I have a roof guy. He came out to reseal some vent stacks. Has an extension ladder with a missing foot, going up 2 stories.
He’s in his 70s. I held the ladder even though he didn’t ask.
Lot of places where the footing doesn't have decent friction. Some sort of anchor is needed and another person can work although it may not be reliable....
Yeah I once had to setup an extension ladder to get attic access through the door hatch, but the carpeting had been ripped up leaving the flooring adhesive. Even with it set at the proper angle it would slowly start to slip down without having someone holding it for me.
2x4 and a couple nails would work if you lacked a trustworthy assistant.
Yeah that's ultimately what we had to do when we both needed to be up in the attic.

Depends on what you mean by not set right. If you place a ladder on a slick surface, like in the video, asphalt or those vinyl decks, once you put enough weight towards the top it can slip under you even if you set it at 3:1. Having it more inclined will help prevent that but it can fall to the side.

Guy at work fell off a ladder at home. He was above a single free standing T post. That's basically a thick steel bar that can be used to hold barb wire fencing.
He fell directly into the bar, it entered his lower abdomen under his ribcage and pierced up through his entire torso so he was hopelessly impaled .
He was stuck like that until someone found him an hour later. He survived but damaged several internal organs and was never the same.

When I turned 55 i determined I would continue to do my own work at home but bought a used scissor lift and I’m done with ladders 4 years now.

I took away my dad's ladder because my mother and I were concerned with him hopping up on the roof to do maintenance. The last time he was up there he lost his footing and took a good bit of skin of his forearm when he fell and slid. After that we said no more.
He's well into his 60s and his coordination and reflexes are just not there anymore. I'll happily pay for someone to take care of his roofing needs if it means he stays off of it. That's like the #1 way people get hurt or die at home.
My grandpa died like this. His last words were "Quit shaking the ladder you little shit!". Still miss him.

The guy just watching could've been the home owner. When I worked installing satelite dishes customers always offered to help or would want to watch me like a hawk the entire time. If they offered to help it was company policy to decline so they wouldnt get hurt. They would stand like this guy watching my every move without really knowing what I was doing. Even if i fucked up they wouldnt be able to tell lol.

Oh yeah obviously, but that's not usually why they are watching. They want to make sure the instillation is done right with 0 knowledge of the technical side of it.
Less about being done right, more about not damaging other things (like tearing off the gutter, putting a foot through a skylight and saying it was like that, etc...)

it was just a sad situation all around and he was only in his mid 30's. from what my mom told me, his coworker wasnt there with him while he was up the ladder when he slipped and fell.

my father in law felt in a similar manner. He broke both of his ankles. He refused to go to the hospital until they started turning black. This was about 3 days after the fall. Once in the hospital they did emergency surgery.

Judging by the angle of the ladder, I assume malpractice at the top. The angle didn't look bad at all. Maybe even too steep. Makes me think the weight at the top (being above the roofline) caused the bottom to slide out.

Has anyone noticed that floor is painted with a very smooth and slippery coating? Not even rubber footpads on the ladder would help. Need bracing or stakes driven into the ground. Tying it on top will help some. But usually on home roofs, there's nothing convenient to tie it off to.
