I have a babydoll style scrub top for work and I thought it was pretty cute until one of patients asked when I was due. Im super skinny and definitely bloat if I have too many carbs but I fully blame the top for being mistaken for being pregnant. I’ve also stopped wearing the shirt after this happened a few too many times in one shift.

It’s a proportion thing. I carry a lot of weight in my chest and midsection so it’s the same on me. Look pregnant no matter what. Even if I lost weight in my midsection there is not enough fabric above the band to support the heaviness on top.

It's a bit odd because some peplum top seams sit between bust and waist, rather than consistently at the waist. But yes, babydoll empire waist is probably more accurate.
